MONTHLY FEE: As stated above. The max monthly family fee will not exceed $350. If paid annually, a 10% discount will be applied. A pro-rated fee will be charged for the 1st month of membership.
ENROLLMENT FEE: A one time non-refundable fee of $100 per member will be charged upon enrollment. The max family enrollment fee is $300.
*Dr. Basil is accepting Medicare patients . We do not bill insurance for our services and still operate on the membership model. Please call for more information*
